We endeavor to work as efficiently as possible and always within the models of best practice. Our interventions and recommendations are made inline with current evidence based practices. We strive to be highly productive with both our time but also with NDIS participants funding whilst ensuring our work is thorough and well thought out.

Our Process

The YCT process ensures we can help you reach your full potential quicker and easier.

 

  • Review types of services we offer. If you are seeking any of the assessments or interventions we offer, continue to step 2.

  • Ensure you have adequate NDIS or Home Care Package funding.

  • Complete referral form or email us directly (home care package referrals) - this will be sent to our team, you will then be contacted (via your preferred method; phone, text or email) to arrange an initial appointment.

    (Currently only accepting aged care referrals)

  • Face-face or tele-health appointment will be had, service agreement signed and your chosen service will be completed. Please be prepared with supporting documents (information regarding disability/ health conditions). Our initial appointment will involve a meeting between participant, nominee and/or support workers/ support coordinator/ case manager. A discussion will be had regarding how your disability/ health condition impacts on your day-to-day life, your goals and necessary standardised assessments will be completed.

  • OT to complete necessary follow up, ongoing appointments, report writing etc. Please note, different services require differing amounts of time/funding. Please refer to our information page regarding our services and estimated hours/funding that will be required.

  • All assessments to be provided to participant, nominee and/or directly to NDIS/ home care provider as per participants consent/ preference.

  • Invoices will be sent out weekly to client if self managed or plan manager if plan managed.

Meet the YCT team

Hannah Warry

Hannah is our senior Occupational Therapist and Director. She is experienced in working with adults with physical, neurological and intellectual disabilities. Hannah has a clinical interest in SIL and SDA assessments.

 

Katie Warry

Katie is a passionate Occupational Therapist with a special interest in working with adults with a physical disability. Her clinical interest is in housing assessments and assistive technology prescriptions. Katie has a warm and easy-going personality and strives to build strong therapeutic relationships with clients to assist them in achieving their goals.
