  • It is expected that all information provided on the referral form and during assessments is true. 

  • Without sighting written documentation that states a person’s disability and/or health condition, our reports will need to reflect that all information stated throughout the report has been “reported by client” and that no supporting documents were provided. 

  • Please ensure adequate NDIS or Home Care Package funding is available prior to OT meeting.

  • OT reports will be provided once final invoice has been paid 

  • Assistive technology prescription, home modifications and general person support recommendations will be made in relation to the participants primary disability or health condition (disability that is recognised by NDIS). We respect that it is often a combination of health conditions that may contribute to a person requiring different support; however all clinical justification will need to be based on the NDIS recognised primary and/or secondary disabilities. 

  • We have a zero tolerance to violence and verbal aggression. Our staff can terminate a meeting at any point these behaviours are displayed.

  • Recommendations made in OT reports are based on a person’s presenting and current level of functioning. Changes to a person’s function may render the recommendation as no longer being appropriate and it is therefore the person/ nominee’s responsibility to contact OT to arrange a follow up appointment.
