PRIVACY POLICY

  • YCT works in accordance with the Australian Privacy Principles. 

  • Your contact details, any supporting documents provided to us, our clinical notes from each appointment and any work completed for you will be stored on our client management system (Nookal).  

  • Personal information provided on referral form will be sent as an email to our central email address and then stored in your client file on Nookal. No personnel outside of YCT has access to this email account or client management system. 

1.   TYPES OF INFORMATION COLLECTED AND HELD

  • Name, DOB, Address, NDIS number, phone number, health records, disability information, NDIS goals, NDIS funding information.

  • This information will be filled in when completing the referral form. Please note, only brief/ vague details are required about your disability. More detailed information will be collected during initial appointment.

2.   HOW WE COLLECT PRIVATE INFORMATION

  • Through the online referral form

  • Through face-face meetings, information verbally provided by participant, plan nominee or support people.

  • Via supporting documents provided (Doctor’s letters, hospital discharge summaries etc.)

  • Liaison with treating health and support team

3.   THE PURPOSES FOR WHICH WE COLLECT, HOLD, USE AND DISCLOSE PRIVATE INFORMATION

  • We require sufficient detail regarding a person’s health and disability conditions to ensure an adequate description and understanding can be had of the functional implication of such.

  • We disclose information that is relevant to our reports and assessments. Information is disclosed throughout our reports that is directly relevant to the recommendation being made.

  • Personal information may be verbally discussed with members of the treating team, but always with consent having been given from the NDIS participant and/or plan nominee.

4.   HOW PARTICIPANTS CAN ACCESS PERSONAL INFORMATION

  • Client or nominated support person can access information once a written request (email) is sent to info@yourchoicetherapy.com.au.

  • Consent is essential
