Client Information and Frequently Asked Questions

    • Your Choice Therapy works in accordance with the NDIS price guide. Any changes to the price guide will reflect changes made to the service agreement.

    • As per the NDIS price guide, a persons NDIS plan will be billed for the following reasons:

      • Face to face appointments with you at your home, OT office or other chosen location

      • Telehealth appointments

      • Phone calls to you, selected family or other involved services

      • Recording session notes, analysing and interpreting assessment results and reviewing relevant documents

      • Report writing

      • Creating resources (as required) to assist with assessment (such as visual aids)

      • Therapist travel to and from appointments

      • Arranging assistive technology trials and seeking and following up on quotes

      • Any liaison between OT, client and necessary stakeholders (builders, AT providers, support coordinators, support workers, LAC, family etc.) This includes emails, text messages, face-to-face interaction and phone call between you and support services related to your care.

    • Your choice therapy will invoice for any of these services mentioned and send this to you if self-managed or your plan manager if your funding is plan managed. The timeframe of payment is 7 days.

  • NDIS:

    Your Choice Therapy bills for travel to and from appointments, in accordance with the NDIS price guide.

    To determine time that can be charged for travel, the Modified Monash Model (MMM) is used. This is a geographical classification system that identifies travel boundaries and sets maximum travel costs that can be billed.

    Your Choice Therapy is based in Sale, which falls into MMM4 – MMM5 regional category. Travel to each appointment and from the last appointment can be charged up to 60 minutes both ways.

    We also charge for the non-labour travel costs, which as specified by the NDIS price guide is $1/per km.

    Home Care Package: As per my Aged Care policies

  • You will be charged a late notice cancellation fee if you do not notify Your Choice Therapy by 3pm the day prior to the scheduled appointment. Your NDIS plan will be billed the standard hourly rate for the time that was allocated for your appointment.

  • Please complete an online referral form and a member of our team will contact you to arrange appointment.

  • Our team will bill using either of the two NDIS line items that OT is listed under. These are:

    • Core - 01_741_0128_1_3 Other therapies

    • Capacity Building - 15_056_0128_1_3 Other therapies

    The hourly rate, as set by the NDIS, is $193.99 per hour.
